Fetal Risk Summary

Bupropion is a unique antidepressant of the aminoketone class that differs from other antidepressants in that it does not inhibit monoamine oxidase and does not alter the reuptake of norepinephrine or serotonin (1). Anticholinergic effects are much less frequent and less severe than those observed with other antidepressants (1). No published reports on the use of bupropion in human pregnancy have been located, but data from a surveillance study and a pregnancy registry are presented below.

Reproduction studies in rats and rabbits at doses up to 15 to 45 times the human dose revealed no definitive evidence of impaired fertility or fetal harm (2). A slight increase in nonspecific fetal abnormalities, however, was observed in two studies with rabbits (2).

In a surveillance study of Michigan Medicaid recipients involving 229,101 completed pregnancies conducted between 1985 and 1992, three newborns had been exposed to bupropion during the 1st trimester (F. Rosa, personal communication, FDA, 1993). No major birth defects were observed (none expected).

A manufacturer's pregnancy registry reported 14 pregnancy exposures that had occurred before the establishment of the reg istry (3). In three of the cases, bupropion was discontinued before conception, and one case was lost to follow-up. Details on the timing of the exposures for the remaining 10 cases were incomplete. The outcomes were two spontaneous abortions (SABs) and eight live infants without birth defects. Over a 3-year period (September 1997 to August 2000), the registry enrolled 236 pregnancies exposed to bupropion that had been reported prospectively (before the pregnancy outcome was known). Of these, the outcomes of 97 pregnancies were pending and 51 had been lost to follow-up. The remaining 88 pregnancies involved 90 outcomes (2 sets of twins). Exposures occurred in the 1st trimester in 65 pregnancies (66 infants; 1 set of twins), in the 2nd trimester in 18 (19 infants; 1 set of twins), and in the 3rd trimester in 5 (5 infants). Among 1st trimester exposed cases, there were 7 SABs, 1 elective abortion, 1 infant with bilateral clubfeet, and 57 live births without reported birth defects. All of the 2nd and 3rd trimester exposures ended with live births with no reported defects. Although only one birth defect was reported, the possibility of additional birth defects being reported could not be excluded (3). (See required statement below.) There were eight outcomes with birth defects reported retrospectively (after the pregnancy outcome was known) (3). Although retrospective reports are often biased toward reporting adverse outcomes, and not normal infants, the defects described were: one kidney, a bicornuate uterus, and double vagina with one opening to outside; unspecified cardiac defects and omphalocele; hypoplastic right heart, atrial septal defect, ventricular septal defect, pulmonary atresia, left transposition of the great vessels, and transverse heart; SAB at 15 weeks' with unspecified congenital heart defect; hypoplastic left heart, diaphragmatic hernia, cleft lip and palate, absence of left radius, hemivertebrae, ear tags, microcephaly, and intrauterine growth retardation; gastroschisis; induced abortion at 19 weeks' due to transverse limb deficiencies, humero radioulnar, intercalary transverse meromelias bilateral, tibiofibular intercalary transverse meromelia of the lower limbs, missing digits of hands bilateral, oligodactyly, imperforate anus, cleft palate, short neck, and intraabdominal calcifications on ultrasound, normal chromosomes, no autopsy; stillborn at 38 weeks', hydrocephalus on visual examination, no autopsy.

Required statement: Committee consensus. After reviewing the 90 prospectively reported pregnancy outcomes, the Bupropion Pregnancy Registry Advisory Committee concludes that this sample is of insufficient size to reliably compute a birth defect risk, and no conclusions can be made regarding the possible teratogenic risk of bupropion. This sample size does not allow for a formal comparison of the frequency of birth defects.

Breast Feeding Summary

Bupropion is excreted into human breast milk. A 37-year-old lactating woman was treated with the drug, 100 mg, three times daily (4). She was nursing her 14-month-old infant twice daily at times corresponding to 9.5 and 7.5 hours after a dose. Peak milk concentrations of bupropion occurred 2 hours after a 100-mg dose with a value of 0.189 µg/mL, but the peak plasma level measured, 0.072 µg/mL, occurred at 1 hour. The milk:plasma ratios at 0, 1, 2, 4, and 6 hours after a dose were 7.37, 2.49, 4.31, 8.72, and 6.24, respectively. Two metabolites, hydroxybupropion and threohydrobupropion, were also measured with peak concentrations of both occurring at 2 hours in milk and plasma. The ranges of milk levels and milk:plasma ratios for the metabolites were 0.093–0.132 µg/mL and 0.366–0.443 µg/mL, respectively, and 0.09–0.11 and 1.23–1.57, respectively. The levels of a third metabolite, erythrohydrobupropion, were too low to be measured in breast milk (test sensitivity 0.02 µg/mL). No adverse effects were observed in the infant nor was any drug or metabolite found in his plasma, an indication that accumulation had not occurred. However, the American Academy of Pediatrics considers antidepressants a class of drugs whose effect on the nursing infant is unknown but may be of concern (5).

i know you take 100mg of bupropion (i aint phsychic) but maybe around 200-300mg a day and might give signs of anorexia if overdose is long-termT_T

But since you are taking the correct requirements, don't worry about anything as it won't give any side effects as long as you watch your taking of Bupropion^_^

Bupropion won't help for weight loss for a girl like you^_^

What can I expect from taking bupropion?
My doctor just put me on bupropion for depression. I am just wondering what anyone else has to say about it. I have heard good things and bad things about it. I want some opinions. Does it work? And how well.

Bupropion is the generic name for Wellbutrin.

I've been taking it for about 8 years now. I'm bipolar, very heavy on the depression and have to take a combination of medication to keep it under control. I've been on 19 different meds and out of those, Wellbutrin is one of the ones that has worked the most with the least side effects for me. It's definitely the best antidepressant I've been on (because of lack of side effects).

When I first went on it, I lost my appetite for a while and had to remind myself to eat. But that didn't last long. The only other time I had any problem with it was when I went from my normal 300 to 450 mg, which gave me hot flashes. On 300 I have no side effects. The first couple of years it was like a miracle, it was so effective. Sometimes over time there meds don't work as well. So I have had a second antidepressant added to the mix at times in order to more fully control the depression. It works differently than the other antidepressants because it works on a different neurotransmitter (dopamine.) Some of the big benefits of it are it has a very low risk of weight gain or sexual side effects, which are extremely common with other antidepressants. I've been on Wellbutrin longer than any other medication..although all of the other meds in my "cocktail" have changed over the years...I stick with this because it works.

My son recently started on it with excellent results at 150 mg. We tried 300 but then he started having side effects that made it clear the dose was too high for him, so back down to 150.

We both are on the XL (once a day) version, but I used to be on SR (twice a day).

Has anyone taken bupropion and chantix at the same time?
I've been taking bupropion for 2 months and today my doctor prescribed chantix as well. Has anyone else taken both at the same time? If so, please tell me details, how you felt, if you recommend it. Thanks in advance.

does bupropion have any affect on the metabolism of methadone?
I know some meds speed up or slow down metaboism of other drugs due to CYP 450 enzymes. I haven't found a website that explains what bupropion does to methadone in the body. any insight into this will be great, thanx.

What is the difference between Bupropion and Budeprion?
Or is it the same thing? (Wellbutrin XL)

If so, why are there two generic names for it?

I ask because previously I had been on 300mg bupropion and my doctor upped the dose by 150mg and when I went to the pharmacy to pick it up they gave me 150mg budeprion. I just want to make sure I'm taking the same thing...

Bupropion is actually Wellbutrin and Budeprion is Wellbutrin XL or SR.

Wellbutrin (not SR or XL) has been available as generic for awhile now, and it's called Bupropion. The SR and XL are both call Budeprion and the XL only just became available as a generic. The SR, sustained release is usually dosed twice a day and the XL is usually only once a day. The regular Wellbutrin I've seen dosed so many ways.

It is the same drug, both are Wellbutrin. The difference I know of is that Bupropion also goes as the brand name Zyban which has an indication to help someone quit smoking.

How does wellbutrin (bupropion) compare to ephedrine, caffeine, etc?
It seems there are a variety of stimulants that boost dopamine, serotonin, and norepinephrine. Though I realize it is unlikely to find an answer here... does anyone have any hard numbers comparing these drugs?

I have taken all 3 and can honestly tell you wellbuterin does not have any effect like ephedrine and caffiene. Whats really funny is that i have taken all 3 at the same time...

I started taking wellbutrin sr (bupropion sr) to heighten my sex drive. How long does it take for it to start working? How long does it take for the side affects to wear off (agitation, nausea...etc)?

Most anti-depressants actually decrease your sex drive, so I'm surprised you would take one to increase it. But then well-butrin is different than other anti-depressants so I guess it's possible it can have the opposite affect. You should start feeling a difference after 2 or 3 weeks, although it could be up to 6 weeks. Good Luck.

How soon after quitting Wellbutrin XL (Bupropion hydrochloride) can I drink alcohol?
I am currently on the lowest dose and am going to quit. I heard you should not drink any alcohol while on this medication, so how soon after you stop taking it is it safe to drink? (I would ask my psychiatrist, but he dropped my insurance, so...)

It would be the safest to wait at least a few days and up to a week before drinking alcohol. Bupropion has a half-life of around 20-30 hours (which means about half of the medication will be out of your body by then). Depending on your metabolism, the drug can linger on for longer. It would probably be safe to have ONE drink starting a day after you stopped the medication, but I would not have more than that after waiting close to a week.

Zyban (BUPROPION)) is a good pill. It is the only pill yet that has kept me from smoking yet. All this herbal and natural c...p is just a scam. If I had more power of will I wouldn't probably even need Zyban.
